Where to document and register all incidents<\/b><\/h3>\n

Depending on the complexity of the incident, learning from it might be as simple as meeting with the team afterward to figure out what went wrong, and what can be done to stop it from happening again. Some incidents may need more investigation to help find out exactly what happened and why.\u00a0<\/span><\/p>\n
